Partner Agency Application
Funding for 2009 / 2010 has been allocated. Applications for the next cycle of funding will be available in March of 2010. Contact the office during that time for an application.
Only non-profit organizations with a 501 (c) 3 IRS status will be accepted. Other documentation such as most recent audit, IRS 990, Board of Directors list, Minutes for the past year, licenses and certifications if applicable and other documents as requested by the Fund Distribution Committee. Deadline for submission of applications: May 1, 2009 before 4 pm.
Funding Cycle: July, 2009 - June, 2010
